Please fill out this application completely and honestly, your approval of puchase is dependent on it. The more information you provide us, the better.

Please understand that by filling out this application you are not guaranteed the approval to puchase a puppy from us nor does it obligate you to do so. We will review this application and respond to you. If we have not replied to your application in a 10 day period, please accept our apologies and understand that life sometimes takes us away from the computer for periods of time.
